    I am currently in the process of credentialing with major insurance companies.

    In the interim, those with insurance can work with me by booking on Fay Nutrition.

    I also provide super bills for patients seeking reimbursement through their insurance.

    Clinical rates for cash pay clients

    75 minute intake session - $175
    50 minute follow-up session - $125
    30 minute follow-up session - $95

  • Cancellation Policy

    Scheduling Policy A $75 fee will be applied for any appointment missed or canceled with less than 24 hours’ notice.
    Grace Period You have a 15-minute window to log in before an appointment is considered a no-show. After 15 minutes, the session will be canceled and the late/no-show fee will apply.

    Please cancel or reschedule at least 24 hours’ notice. Since I can't determine what qualifies as an emergency on a case-by-case basis, late cancellations will incur a fee to be fair to all clients.

Good Faith Estimates for Uninsured or Self-Pay Patients

Under federal law, you have the right to receive a "Good Faith Estimate" of expected charges for healthcare services. If you don't have insurance or choose not to use your insurance, we will provide you with an estimate of the cost of your care before you receive services. You may also request a Good Faith Estimate for any scheduled service by contacting us directly. We will provide this estimate within 1 business day for scheduled appointments or 3 business days for other requests. The Good Faith Estimate will include expected charges for the healthcare services reasonably expected for your care. If you receive a bill that is at least $400 more than your Good Faith Estimate, you have the right to dispute the bill.
